Addressing social determinants of health (SDOH) is essential for reducing health disparities and achieving health equity, and health centers are uniquely positioned to lead this effort. Join us for a conversation with Dr. Cheryl Clark, Executive Director and Senior Vice President of the Institute of Health Equity Research, Evaluation and Policy at the Massachusetts League of Community Health Centers, and Co-Chair of the Social Determinants of Health Task Force of the National Institute of Health's All of Us Research Program. Dr. Clark will review the paths through which social determinants affect health and explore the history of the nation's community health centers to reveal why and how they can effectively reduce health disparities and promote true health equity among the most vulnerable patient populations. We'll also provide a framework for identifying and understanding health inequities at health centers utilizing our report, A Data Driven Approach to Understanding Factors Contributing to Health Inequities, based on an analysis of the health center in Los Angeles County, CA.
