Supported by a Cooperative Agreement with the Health Resources and Services Administration, Bureau of Primary Health Care, this report is based on the results of a 2014 assessment conducted to determine the capital plans and needs of health centers across the US. The assessment also gathered information on funding sources and interest in collaborative projects addressing the social determinants of health.
