This report is based on an assessment conducted in 2012 to determine the capital plans and needs of US community health centers and assess whether those plans, if funded and implemented, would be sufficient to provide the facilities necessary to serve 40 million patients, the Affordable Care Act goal for access to care at that time.

The assessment also gathered information on funding sources and interest in collaborative projects addressing the social determinants of health.
